- Application for the combination BA/BS and M.Ed. program Use this form to apply for admission to the 5-Year program.
- EDUC 114-115 Attendance Verification Form Use this form for documenting the hours that you observe in the classroom. This form must be signed by you, the teacher, and the principal at your observation site.
- EDUC 114-115 Rating Form for Teachers and Principals Principals and teachers use this form to give feedback on students who are observing classes in Early Field Experience.
- EDUC 114-115 Placement Information for Scheduling Provides information for the EDUC 114-115 instructor to use for placing students in classes for observation.
- Education Practicum Application - must be filed when preregistering for Education 401.
- Exit Interview Review Memorandum - You must complete this form at the Exit Interview after you complete student teaching. The form updates your contact information, gives your current status on required Praxis tests, and has information useful to you as you exit student teaching. You may download the form and complete it prior to the exit interview and turn it in at the interview. Be sure that you keep a copy of your signed form for your records.
- Mentor Contact Information - Practicum and student teacher mentors use this form to update their contact information.
- Praxis Registration: You may register on line or download a form to register by postal mail. Other forms for special accommodations and changes of registration dates or tests are available on the Praxis web page. On the Praxis website, choose "Download" from the link bar for a list of downloadable forms, study guides, and information. Other tabs lead to additional information about test dates, locations, etc. If you have difficulty navigating the Praxis website, come by the Neff Center office for assistance.
- Program Interview forms All students in the Teacher Education Program must have a Program Interview prior to student teaching. Usually this interview is in the first semester of the Junior year. Please consult your advisor about the scheduling of your Program Interview.
- Student Teaching Application - must be filed when preregistering for the student teaching semester.
- Practicum Handbook - In the Practicum semester, this book is a useful source of information and forms for students, mentors, and faculty advisors.
